A Renfe AVE train struck the rear of a stationary Iryo train between Mora and Orgaz on Wednesday, causing several carriages to derail, with some plunging down an embankment.
Transport Minister Oscar Puente said a computer communications failure automatically halted several trains, but a central command centre then gave an “inexplicable” order for the Renfe train to proceed at normal speed.
Investigators are seeking to establish why controllers appeared unaware that a train remained stalled on the tracks ahead.
The disaster has sparked political criticism, with the opposition demanding answers over rail safety failures, while Prime Minister Pedro Sanchez has pledged a full investigation and an urgent review of safety protocols.
King Felipe VI and Queen Letizia visited survivors in hospital in Toledo as Spain held a national minute of silence for the victims.
